Personnel identity card

1. Procedures for the issuance of personnel identity card 
Step 1: Application form
Personnel contact officer at the Human Resource Management Office, 4th Floor of Utility Exchange Building, or call 02-4708084 to obtain and submit personnel ID card application form. 
Step 2: Photography session
Personnel have their photograph taken at the Student Registrar’s Office, 1st Floor of the Office of the President Building, Monday to Thursday between 8.30 a.m.–4.30 p.m. or submit a 1-2 inches photograph (taken within the last six months) to officer on the date of application form submission. The photograph can send via e-mail to Kmuttcard@kmutt.ac.th.
Step 3: ID Card pick up and return
Personnel ID card can be picked up/returned at the Student Registrar’s Office, 1st Floor of the Office of the President Building, Monday to Thursday between 8.30 a.m.–4.30 p.m., or at the Human Resource Management Office, 4th Floor of Utility Exchange Building, Tuesday, Wednesday and Friday between 8.30 a.m. – 4.30 p.m. Call 02-4708084 for more detail
2. Dress code for personnel ID card photograph
(1) University officers:
Men: A black or navy blue blazer or suit with a plain white/cream shirt and black/navy blue necktie or a regular white uniform
Women: A black or navy blue blazer or suit with a plain white/cream shirt or a regular white uniform
(2) University employees, supplement officers, advisors, advisors to president, University board:
Men: A black suit/navy blue blazer or suit with a plain white/cream shirt and a black/navy blue necktie
Women: A black or navy blue blazer or suit with a plain white/cream shirt
(3) Civil servants  in higher education institute: 
A white, regular ceremonial attire, a university jacket or a Thai Praratchathan attire (Royally-initiated style of Thai attire)
(4) Permanent employees: 
A white ceremonial attire (white formal uniform, half-dress uniform, full-dress uniform), an operation uniform or a university jacket 
(5) Retired civil servants:
A white, regular ceremonial uniform with a “Nor Gor” insignia affixed to the jacket’s right collar and a department insignia affixed to the jacket’s left collar.
(6) Muslim female officer:
when wearing a headscarf (hijab), wear a modest black headscarf with a plain edge to cover the head except the face.  The headscarf covers the shoulders at both sides while in the front reaching the chest level. In case of wearing a regular white uniform, the headscarf edge is tucked under both front and back collars. If wearing a hat, wear it over the headscarf
3. Validity period of personnel ID card
Personnel ID card shall be valid for a period of 6 years from its issuing date, with the exception of:

  1. However, the ID card of retiring personnel who are appointed to an administrative position shall continue to be valid for the period of their appointment.
  2. After reaching 70 years of age, the ID of retired civil servants in a tertiary educational institute shall be issued with a lifetime validity. 
4. Upon ID card expiration and personnel’s resignation from work, do they have to return their ID card? 
  1. In case of, Personnel must immediately return their expired ID cards to responsible officer on the day that they pick up a new ID card. 
  2. Upon termination of their university personnel status, personnel must return their ID card to their affiliated work unit for delivery to the Registrar Office or they may return it to the Registrar Office in person.
5. In case of loss of personnel ID card,
  1. Personnel must report such loss at a police station for the issuing of a police lost document case report;
  2. Present the police lost document case report to the Treasury Office and pay a fee of 200 baht for the issuance of a new ID card (no fee is required in case of theft).
  3. Contact the Registrar Office to obtain an ID card application form. Submit the police lost document case report and fee payment receipt issued by the Treasury Office.
6. Available services for personnel ID card holders 
  1. Inter-campus coach service
  2. Library’s book loan/return service
  3. Parking space under buildings and in the 14 Floors Car Park Building (parking sticker required. Contact the Office of Building and Ground Management for a sticker).
  4. Entrance and exit to and from buildings on campus.
7. Causes for change/reissuance of new personnel ID card
  1. Expired card
  2. Lost, destroyed or damaged card 
  3. Change of work position, first and last names.
